This year’s flu clinic will be held on October 22. Participants will need to make an appointment to receive the vaccine. Please fill out and bring the attached consent form and a copy of your insurance card to your appointment. The 65+ flu vaccine can be selected when you make the appointment. The COVID vaccine will not be provided.
Please use the links below to schedule an appointment at your building. Call 518- 886-8251 (ADK Healthcare) to reschedule your appointment or to see if an additional spot is available if the dates are greyed out.
- Click the link and it will take you to the Adirondack Health & Wellness page, select the dropdown arrow under SELECT SERVICE
- Choose which service you are receiving (FLU)
- This will take you to the date the clinic is offered, which will appear in white. (Grey dates are unavailable)
- Next select your preferred appointment time (which will appear in white, grey are slots unavailable)
- Fill out Name, Contact#, Email, DOB, Health Insurance Company, Insurance Member ID# & Group#.
- Select the option to have an appointment reminder sent and then finalize the appointment.
- Appointment is confirmed. You can print your appointment details from this screen.
- Please plan to arrive at the appointment a few minutes early with a copy of your insurance card. There is a short form that is required prior to services rendered.
